Steps for CONNECTICUT Patients Registration

  • To utilize the online certification system, patients must first register with the DAS Business Network.
  • Sign up for a free account at https://biznet.ct.gov/dcpmmrp

**Please Note: Before registering, make sure your physician has certified you.**

  • Follow all prompts and instructions to guarantee a successful account creation and verification.
  • Then you may log in and start registering after verifying your account via email.

Log In

  • Log in with your DAS Business Network email address and password.
  • If you choose “I am a patient,” the system will ask for the following information:
    • E-mail Address
    • Date of Birth
  • Examine the Patient Information webpage for accuracy.

Required Documentation

  • One of the following:
    • Proof of Identity
    • Proof of Residency
  • Certified: Patients must read and confirm each statement on the website.

Payment

  • The cost is $100.00.

Make check or money order payment to:

**The Department will not accept cash payments.**

  • To create a free account, visit https://biznet.ct.gov/dcpmmrp.

**Please Note: Make sure you have been certified by your physician before beginning the registration process.**

  • Follow all prompts and instructions to guarantee a successful account creation and verification.
  • Then you may log in and start registering after verifying your account via email.

Log In

  • Log in with your DAS Business Network email address and password.
  • If you choose “I am a patient,” the system will ask for the following information:
    • Birthdate Mailing
    • Address
  • Examine the Patient Information webpage for accuracy.

Caregiver Information

  • Depending on the patient’s medical state, the doctor may authorize a caregiver. Applicants must submit a valid email address and complete the caregiver information section.
  • The caregiver must do the following:
    • Manage a registered patient’s palliative marijuana use.
    • Not be the patient’s doctor;
    • Unless a parent, guardian, conservator, or sibling relationship exists, only be responsible for one (1) patient.
    • Act as a parent, guardian, or legal custodian for an adult patient.

Required Documentation

  • Select and upload one (1) of the following:
    • Proof of Identity
    • Proof of Residency
  • To be certified, patients must read and accept all statements on the website.

Payment

  • The patient application fee is $100.00.

  • For online certification, the caregiver must first register with DAS Business Network. To register, click here: https://biznet.ct.gov/dcp-mmrp
  • Follow all prompts and instructions to guarantee a successful account creation and verification.
  • Then you may log in and start registering after verifying your account via email.

Log In

  • Log in with your DAS Business Network email address and password.
  • When you choose “I am a Caregiver,” the system will ask for the following information:
    • Birthdate
    • Mailing Address
  • Examine the Primary Caregiver Information webpage for accuracy.

Required Documentation 

  • Select and upload one (1) of the following:
    • Certifications for Proof of Identity: Caregivers must read and confirm each statement on the website by clicking “Yes.”

Payment

  • The caregiver application fee is $25.00.
